A question about H5N1 in Indonesia

Yesterday the Indonesia Ministry of Health updated their site with a new H5N1 bird flu death. This is the 9th death reported in Indonesia this year. 
The case fatality for 2012 is 100%. This is not a believable statistic. How many cases are not being reported to the World Health Organization?
